Also, you may prefer payment by Credit Card. PLEASE include the zip code of the card owner. We are unable to process your registration without this information. We are accepting Visa, MasterCard and Discover Card.

Each team is automatically entered into the Team Award System. A single Team Entry Fee is assessed PER gym. This covers all team entries as well as many other expenses such as the coach’s room and more.

If you have more gymnasts than can fit on page 1 and 2, simply copy page two as many times as you need.

You may fax the form to STGA at 607-754-7484. You may pay for your registration by credit card or check. Please include credit card information on the entry form that you fax. If you choose to fax and pay by check, please mail the check payable to:
